Organon's $954M bet on the non-hormonal endometriosis frontier collapsed when Forendo (FOR-6219) failed Phase 2 in mid-2025. It was not the first non-hormonal mechanism to die in the clinic. Eliapixant, BAY1128688, and linustedastat preceded it. The attrition rate in this class exceeds 60%.

What remains is a competitive vacuum. HMI-115, Hope Medicine's anti-prolactin receptor antibody, is the only non-hormonal endometriosis therapy in Phase 3 globally. It is also the only one with Lancet-published Phase 2 efficacy data: 42% reduction in menstrual pain and ~50% reduction in non-menstrual pelvic pain, with no menopausal side effects and no bone density loss. It holds both FDA Fast Track and NMPA Breakthrough Therapy designations.

The nearest non-hormonal competitors sit in Phase 1–2, most without disclosed human pain efficacy. For a pharma BD evaluator building a non-hormonal endometriosis thesis, the pipeline map is stark: one asset at the front, a graveyard behind it, and a 2–4 year gap to the next entrant.

The deal-comp evidence reinforces the leverage. Pharma has paid $875M (KaNDy/Bayer at Phase 2), up to $4.2B (Pfizer/Myovant at Phase 3), and $954M (Organon/Forendo at Phase 1) for endometriosis and women's health assets. The competitive vacuum compresses the field into a single partnerable asset at the most deal-ready clinical stage.
