This report maps the global radiopharmaceutical competitive landscape across ~2.3K active programs as of Q3 2026. Roughly 75% are diagnostic imaging, with therapeutics concentrated among radioligand therapies. Gen-1 RLT (Pluvicto, Lutathera) is already a multi-billion-dollar commercial market, while the next wave of new targets and alpha emitters remains early-stage. China has become the largest originator (~1.0K of ~2.3K active programs) but the assets skew early and mirror Western composition.
Beyond PSMA and SSTR, therapeutic targets sort into crowded (FAP, CAIX), contested (GRPR, DLL3), open (GPC3, MC1R as alpha bets), and re-entry plays (HER2, B7-H3). Approved RLTs are all Lu-177, with Ac-225 and Pb-212 leading the alpha emitter wave. Alpha superiority is not yet clinically proven, and Ac-225 supply is a bottleneck with major players locking up current capacity.
Deal value is on pace for a record year at ~$13.5B run-rate, increasingly driven by supply-chain deals. Novartis built its franchise through more than $7.5B in asset M&A, then locked its own Ac-225 supply, manufacturing, and radiopharmacy. Buyers are consolidating every link: large pharma buys drug assets, specialists build integrated stacks, and private equity buys the infrastructure. ~92% of deals are intra-Western.
The binding constraint has moved from manufacturing supply in 2022-23 to delivery infrastructure today. Nuclear-medicine chairs, limited radiopharmacy staffing, and reimbursement friction cap RLT growth, as no molecule can fix a shortage of treatment slots. China originates the pipeline but its radioactive supply chain cannot cross borders. We expect supply-chain control to decide who stays independent, alpha timing to set who competes beyond Lu-177, and delivery infrastructure to cap the commercial trajectory.
